Procedures for Disclosure

We will respond properly to inquiries concerning requests for the notification, disclosure, correction, etc., and the suspension of use, etc. of the personal data of customers in accordance with the Act on the Protection of Personal Information (hereafter, the "Request for Disclosure, etc.").

1. How to Make a Request
Please contact the contact point below. We will send our prescribed request form to those who intend to make a Request for Disclosure, etc. Please complete the form and return it to us at the designated contact point. Please also submit the following documents:

(1)For requests made on the person’s own behalf
  • A copy of a document issued by a public institution, such as a driver's license, passport, health insurance card or pension account book.
(2)For requests made by a proxy
A document that permits identification of the proxy (same as the documents in (1) above). In addition,
please submit the following documents:

  • For a statutory proxy, a document that confirms the statutory proxy status, such as a copy of one’s family register, or a copy of certificate of registered matters concerning adult guardianship
  • For a privately appointed proxy, a Letter of Proxy and seal registration certificate of the actual person.

2. Fee
We will charge \700 (including tax) for sending documents, etc. as our response to a request for notification of purpose of use or disclosure of retained personal data. Please remit the fee to the bank account designated by the Company.
Please note in advance that the customer shall bear any postage charges incurred for sending the form for Request for Disclosure, etc. to the Company, and any remittance charges incurred for remitting the fee.

3. How We Will Respond to Requests
Once we have received a request, we will confirm and investigate the details of the request. After confirming the receipt of fees for fee-based requests, a response will be made to the person in writing. When the request has been made by a proxy, the response will be made to that proxy.
We may be unable to respond to a request if a response to a Request for Disclosure, etc. is likely to harm the life, body, property, or any other rights or interests of the person in question or a third person, seriously impede the proper execution of our businesses, or violate other laws or ordinances. In this case, we will respond with an explanation as to why we cannot fulfill the request.
